About this service
Matt Talbot Housing includes both transitional housing and long-term supportive housing.
Our transitional housing offers stays of up to one year. Clients work with our case coordinators on an integrated care-plan that guides a process of gradual reassertion of autonomy and self-direction through one-on-one support and other therapeutic interventions, strengthening positive routines through enhanced contributions to community development. Admission to transitional housing may come from Residential Treatment, Extended Residential Treatment, or by referral from an external agency. Transitional housing clients can extend the trajectory of support by accepting admission into our Wrap-Around Aftercare program. We are currently accepting applications for admission to our transitional housing.
