DECNET, the Diabetes Education Community Network of East Toronto, is a diabetes education program that provides counselling, education and support to help adults manage their type 2 diabetes or prediabetes.
SERVICES OFFERED:
- One-to-one appointments with a diabetes educator (Dietitian, Nurse or Social Worker) Learn more about what diabetes educators do
- Group Education (e.g. nutrition, insulin, physical activity, stress, etc.) Learn more about group education classes offered
- Support, Advocacy & Outreach Programs (e.g. mental health and diabetes, language specific support groups, and our advocacy group) Learn more about the support groups offered
Who can use it?
Services are also available for individuals at risk of developing type 2 diabetes.
