Our Approach In many communities, there are limited options and resources for individuals who are transitioning out of hospital, awaiting a long-term care space or needing healthcare and daily living supports in their residence. Our unique approach starts with the needs of the individual. We develop a care plan for each individual, assembling clinical resources and determining the best venue of care with the individual, funder, and/or family in order to promote the best outcomes for the individual. The delivery of service could be in one of our community based homes or in the individuals personal or family residence. Support in Our Community Homes and Personal Residences Our specialized community services are designed to support individuals in attaining their optimum level of independence and self-reliance by developing life skills and activities of daily living. We offer an interdisciplinary care model that provides seamless transitions along the continuum of care, and sustainably achieves positive health outcomes. Our specialized community homes provide a wide range of outcome-focused supports for children, adolescents and adults with special needs. Located in residential neighbourhoods throughout Canada, our homes offer 24/7 services, our homes are equipped to cater to individual medical and behavioural needs. We support individuals with a wide range of needs including: Developmental disabilities Physical disabilities Acquired brain injury Autism Medical fragility Behavioural challenges Cognitive deficits Mental health challenges Complex care needs Respite care needs Family crisis stabilization
