About this service
Identification is necessary to apply for subsidized housing and income assistance programs (such as Ontario Works), receive appropriate health care and access many other programs and services in Canada.
The health card kiosk and identification replacement clinic is a joint partnership with Street Health Community Nursing Foundation and Ministry of Health and Long Term Care. Hosted on the fourth Wednesday of every month, this clinic helps homeless people replace lost, stolen or damaged identification - usually within 3-6 weeks.
Staff from Ministry Of Health are on site to process health card applications. Staff from Street Health are available to process birth certificates, record of landing (ROL) documents and social insurance number cards for homeless people.
