About this service
The Hepatitis C Program at Scarborough Centre for Healthy Communities is part of the Ontario Hepatitis C Network. The program offers prevention and care services for individuals at risk of or living with Hepatitis C. Services include community-based point-of-care testing, education, linkage-to-care, treatment, and mental health support through a wrap-around care model. The interdisciplinary team comprises a Nurse Practitioner, Harm Reduction Worker, Ambassador, Social Worker, and Medical Secretary. The program focuses on a client-centered approach and involves collaboration with the community and individuals with lived experience.
