About this service
The Diabetes Education Program at Scarborough Centre for Healthy Communities provides comprehensive diabetes care and education for individuals aged 18 and older, including families and new immigrants. The program, free of charge and accessible to those without insurance, covers blood sugar monitoring, medications, healthy eating, foot care, physical activity, stress and weight management. Clients can receive support in multiple languages including English, French, and several South Asian languages. The program involves a team of healthcare professionals, including Registered Nurses, Dietitians, Social Workers, and offers endocrinologist consultations and foot care services. Self-referrals are welcome.
