About this service
We support participants in any way that will encourage them to be comfortable in their homes and in the community at large. Our team consists of two full-time Follow up Support Workers and several Peer Support Volunteers.
We are able to assist participants in many ways. Sometimes, it is as simple as providing a list of local food banks during a home visit. Frequently, we are able to help with completing Toronto Housing Connections applications, obtaining necessary identification documents or health care, supporting someone through court proceedings or assisting with welfare and Ontario Disability Support Program (ODSP) applications.
The program operates largely out of a minivan and transportation is provided to our Participants, to appointments, moving, and other opportunities. Our support involves rent checks, orientation to housing and the neighbourhood, connection to medical, dental, Identification and legal services.
