About this service
Bellwoods has been delivering various types of transition programs and services for several years. However, the profile of Bellwoods work in this area was heightened in November 2017 when Bellwoods was selected to participate in a pilot project funded by the Ministry of Health and Long-Term Care and Toronto Central LHIN to test Short-Term Transitional Care Models (STTCMs) focused on supporting the transition of alternate level of care (ALC) patients from hospital.
Today, Transition Programs offered by Bellwoods include:
Community Connect (CC) Program (15 reintegration units)
A Centralized Referral Management Team providing a single point of referral and triage for all of the Short Term Transitional Care Models being pilot-tested in the TC LHIN
Caregiver ReCharge Services (CRS) that provides in home respite for caregivers of clients who are transitioning from hospital
