About this service
Our team of Primary Care Providers, Physicians and Nurse Practitioners, provide medical care to individuals of all ages. This team is comprised of 3 part-time Physicians, 3 full-time Nurse Practitioners, a Clinic Nurse and an Adult-focused Mental Health Therapist.
Services include: Prenatal Care, Well-baby Assessments, Immunizations, Cancer Screening Tests, Chronic Disease Follow Up, Collaboration with Specialist Services, Health Incident Management (i.e. infections, bronchitis etc), Mental Health/Addictions Counselling and Coordination of Care for Elders.
