About this service
Your care team.
We will set you up with a team of highly skilled professionals, depending on your needs. This might include case managers, physical and occupational therapists, speech language pathologists, social workers, psychologists, psychiatrists, family doctors, etc.
Your program.
Your care team will create a program tailored just for you and we will help you follow it. Your program will address all the aspects of independence, including any cognitive, emotional, social, physical, spiritual and behavioural issues you have.
Your living requirements.
We offer several models of support for daily living, which include a combination of rehabilitation and/or personal care services. This can be provided in your home. Alternatively, if you need it, March of Dimes Canada can set you up to live independently either in a group home or in a supported apartment complex.
