Person Centered Planning (PCP) is a value-based process that helps people who receive support have opportunities to reach their full potential, to be fully included in community in ways they find meaningful, and to enjoy the rights and responsibilities of full citizenship. In essence, it is a process that empowers people supported to achieve their definition of a good life. The cornerstone of this approach is that it is individualized, and it values and celebrates the uniqueness of the person receiving support. Person Centered Planning focuses on the person's strengths and abilities, and helps the person have meaningful experiences to determine and realize their desired lifestyle. Person Centered Planning strives to help people who receive support have a lifestyle that is rich in experience, inclusive in nature, and has ongoing opportunity for personal growth. Person directed goals are the result of this process.
