About this service
COVID 19 Update: Offices are open, telephone and virtual visits are available for most other appointments.
Services
- The Diabetes Care Team is for any patients living in the Niagara Region with Type 2 diabetes or Pre-diabetes
- The Diabetes Support Program is strictly for Niagara North Family Health Team patients of the Virgil and/or Niagara-on-the-Lake sites.
- Individual assessment and counseling is provided to help you work towards a plan that allows you to control your diabetes and prevent its complications.
- Ongoing follow up support for clients who require more individual support for their self-care management is available.
Physical Activity Class (Virtual)
- learn from a Registered Kinesiologist about the importance of physical activity in the prevention and management of diabetes
- receive a resistance band and be given tips and guidance about how to effectively utilize it in their daily exercise routines
- Program registration form available online
