About this service
What we do?
- Work with individuals to assist them to live safely, securely and independently in the community
- Assist individuals with accessing and maintaining services and supports in the community
- With a persons consent and direction, explore options with individuals to assist with decision-making and goal setting
- Promote Social Inclusion
- Assist with Coordination of Services
- Person Centred Planning
- Work in partnership with a variety of community agencies to improve the economic and social conditions of adults with developmental disabilities
To Qualify for Service You Must:
- Be a resident of Niagara Region
- Be at least 18 years of age
- Have a confirmed developmental disability
- Voluntarily request service
- Live independently or be moving towards independence
- Be referred to the program by Developmental Services Ontario Hamilton Niagara Region (DSO HNR)
