About this service
The name Congestive Heart Failure (CHF) is somewhat misleading, as it does not mean your heart has failed. It means your heart doesn't pump as strongly as it should because the heart muscle is weak.
Through home and office visits, and phone support, the program nurses assist patients in understanding:
- the signs and symptoms of CHF
- the importance of medication compliance
- diet and fluid restrictions and
- importance of exercise
The CHF Program also has a direct affiliation with GHCs Cardiac Rehab Program. CHF patients can be referred to the exercise component (VIPEX) as well as the services of the registered dietitian and social worker.
