About this service
In 1990 Pathways to Independence was created to support the transition of nearly 100 people with developmental disabilities from long term institutional life to community living. With over twenty five years of experience of providing community based services and supports, Pathways has earned a reputation with our community partners, funders, family members and other stakeholders as an agency that provides leadership to identify and address evolving service needs for adults with developmental disabilities.
Our Quality of Life Planning (QLP) process helps to identify interests, skills and abilities for each supported person. The result is a service plan, goals and objectives that clearly identifies what is important to a person, what they want to learn, do or achieve, and has clearly identified steps to help them to achieve their goals.
