About this service
The Housing Based Case Management program is accessible to youth ages 16 to 24. Housing Based Case Managers (HBCMs) provide individualized supports in order to assist homeless and/or at-risk youth in obtaining stable, safe, affordable, long-term housing. HBCMs will also refer clients to other resources that support housing retention. Referrals would include, peer support, education, employment, mental health, substance abuse and recreation. HBCMs work with clients on an ongoing basis to assess and support clients in gaining knowledge, tools and skills to live independently.
