About this service
Hypertension
A comprehensive program provided by our Nurse Practitioners involving screening, diagnosis and treatment of hypertension. Particular focus is placed on risk factor screening and reduction. Health promotion, including healthy lifestyle counselling is provided by Nurse Practitioners.
Diabetes
An evidence based program focused on educating and empowering our patients in managing their diabetes. Each appointment will combine both education and counselling provided by our experienced nurses as well as treatment optimization by your doctor. Review of your glycemic control as well as managing and preventing common complications of diabetes will be accomplished through a team based approach.
