About this organization
The Champlain CCAC connects people of all ages with the health and support services they need to: 1) remain safe and independent at home, 2) return home safely from hospital and avoid readmission, 3) find primary care, and 4) explore long-term care options,day programs, and more. Our goal is to connect people with the right care at the right time and in the right place. Our highly, qualified health professionals provide a range of specialized care at home and in the community, including physiotherapy, nursing, speech therapy, palliative care and more. We work with people of all ages to ensure they make informed choices about their care and receive services in the most appropriate settings.
