About this service
The Collaborative Residents Enabling Assisted Transitional Engagement Program (CREATE) is a Transformational Housing Model offering high support housing in a collaborative partnership with Ontario Shores Centre for Mental Health Sciences.
The program provides intensive 24/7 onsite support for successful community reintegration of Ontario Shores clients. Clinical supports are provided through Ontario Shores and CMHA Durham to facilitate transitions to the community through safe, supported housing.
