About this service
The Case Management Program provides supportive and flexible outreach support services to individuals who have been diagnosed with a serious mental illness with the goal of promoting wellness, independent living and reducing the frequency and duration of hospitalizations. Through community based moderate to intensive case management, workers provide support to ensure that an individuals rights, dignity and independence are maximized.
