About this service

Supervisors, Client Services are responsible for overall care coordination of Peel Senior Link clients as well as those that are referred to our supports for daily living program. Our staff use their expertise to determine client care needs and establish a client-centred plan of care to restore/maintain health and independence within the home. This requires collaboration with families and caregivers, hospitals, primary care physicians, long-term care homes and other home and community support services.
