About this service
- Health Care Connect is a programme provided by the Ministry of Health and Long Term Care
- The programme refers people without a regular family health care provider to physicians and nurse practitioners who are accepting new patients in their area
- It is a voluntary programme for both patients and participating family health care providers
- Patients can register by telephone with a centrally located patient representative, or, they can register on-line
- Care Connectors are based locally to facilitate the connection between patients and doctors/nurse practitioners. There are Care Connectors in each LHIN throughout the province
- Patients must have a valid Ontario Health Care Number to be eligible for the program
