About this service

The program focuses on providing community-based rehabilitation and support to individuals living independently in the community who do not require the same level of support offered in ABI specialized assisted living, long term care, or group home settings.

Clients accepted into the program participate in an Independent Living Assessment which evaluates a variety of domains such as: cognitive, memory, attention, language, social, ADLs/IADLs, etc. The results of this assessment help to identify the specific rehabilitation needs of each individual, which are then taken into consideration while developing their rehabilitation program.
