About this service
1to1s Social Workers act as care coordinators to assist families with navigating health care services within and outside of 1to1 Therapy Services. A care coordinator is assigned to each client at 1to1 at no cost. When you phone 1to1, a Social Worker will complete your intake over the phone and make a recommendation for service including an individualized clinical team. For clients with or suspected of having a Neurodevelopmental Disorder or Autism Spectrum Disorder, a no cost in person consultation will be offered. Diagnosis is not required. Based on the outcome of the consultation, recommendations will be made for service and a personalized clinical team will be created. Our Social Workers work to coordinate care with the client or family. They can provide guidance on services, community resources, finances, funding options and much more.
