About this service
The goal of the Outreach Worker/s is to be out in the streets and community, where the youth are, and to provide for their immediate needs. Outreach worker/s also direct youth to other services (both ours and those provided in the community). Intensive Case Management Worker/s - which is our Streets to Housing team, help youth who are struggling to find and maintain housing under the Housing First Model. The Intensive Case Management worker/s also help youth resolve conflict within their immediate family, connect with their extended families, and prevent youth from ever becoming homeless. Services offered include: individual and family counselling, family mediation, case management and support and connecting people to local community resources.
