About this organization
The South East Community Care Access Centre (South East CCAC) provides one-stop access to health and personal support services to help people live independently in their homes or to make the transition to a long-term care home. Health care services provides to eligible clients include: nursing, personal support support services (such as assistance with bathing & dressing); physiotherapy; occupational therapy; speech-language therapy; social work; nutritional counselling/dietetics; medical equipment and supplies; access to drug coverage; and respite services. The SECCAC also connects people with information and additional services that may be available in their communities such as: adult day programs; in-home assistance; meals-on-wheels; food delivery; palliative care; bereavement counselling; telephone assistance and support; transportation and attendant outreach services; as well as providing assistance with finding a family health care provider.
