About this service
The purpose of this crisis support and intervention is to provide brief crisis follow up, aiming to ensure that individuals discharged from hospital are linked to appropriate community resources. Based on the needs of the individual, this program aims to reduce the amount of times an individual presents in crisis at the Hospital through appropriate community supports.

Referrals from Peterborough Regional Health Centre and Ross Memorial Hospital.
