About this service
Easy access to a range of community services is vital for allowing you or your loved ones to cope with health or social issues while continuing to live independently. St Josephs Home Cares Care Coordination service will help you navigate the health care system and link you with the necessary services. In addition to offering a range of services provided by our own Nurses, Personal Support Workers, Companions, and Cleaners, we can also connect you with other types of health and community supports that will enable you or your loved ones to remain living at home independently.
