About this service
A unique day program helping people return to independent living at home after early discharge from acute care, rehabilitation or convalescent care programs. Many people are better served by continuing their recovery in their own homes with support. This ambulatory convalescent program, partnered with enhanced CCAC services, will allow you to continue your convalescence from the comfort of your own home. Your program will include nursing, physiotherapy, recreation and other support to help you rebuild strength and regain skills to reduce your need for assistance.
The program is funded by the Ontario Ministry of Health and Long Term Care. There is no cost to participants.
