About this service
The Homelessness Prevention Case Manager (HPCM) strives towards homelessness prevention for youth, aged 16-25 years, who are street-involved and/or homeless to (re)transition off the streets, out of unfit and unstable housing, into appropriate and stable housing opportunities. The HPCM assists with obtaining a housing placement and providing case management support for youth to maintain housing. Case management is focused on providing support for youth to find, obtain and maintain safe, appropriate and affordable housing is available. The HPCM offers assistance conducting housing searches, attending housing viewings, building relationships with landlords and navigating financial resources and programs, such as Ontario Works and Ontario Disability Support Project, which provide an income for rent and other housing related costs/needs and help participants move into their housing.
