About this service
Care Coordinators are trained health-care professionals who can assess patients and provide advice on what services will best suit your needs. Your specific care plan is completely individual and geared towards achieving your optimal health and wellness at home and in your community.
Your home care team will identify the supplies you need to support your care and have them delivered to a depot location chosen by you at no cost to a depot in your region. Your Care coordinator or nurse will share the various options for Depot locations available to you and also provide information about options for direct delivery to your home.
