About this service
Intensive Case Management for Seniors at Risk (ICM) provides intensive case coordination with an in-depth, personalized, flexible, and creative approach. This program specializes in elder abuse, including physical, psychological/verbal, financial, neglect and/or self-neglect. Anyone may call to make a referral. The service is free and confidential and is for clients 60+ years of age, residing in Hamilton.
Case coordinators offer home visits, where an assessment of the clients situation is completed; then options are discussed
Consultation offered via telephone for families/caregivers who do not wish to receive a home visit.
Cultural differences and personal preferences are always taken into consideration to preserve autonomy
Goals:
- improve the clients quality of life by utilizing the least intrusive and most accommodating approach
- assistance and support provided to seniors so they may live independently in the community
- provide education, awareness and intervention regarding elder abuse
