About this organization
The Hamilton Family Health Team is the largest Health Team in the province. There are 166 family doctors and over 250 other health care workers in the Hamilton Family Health Team, and we serve more than half of the people living in the City of Hamilton. Our practices are in many locations across Hamilton. Some have several doctors in one location, while others have only one or two family physicians, but they all work with a team of healthcare professionals to provide you with the best care possible.
In addition to giving you one-on-one care, the Hamilton Family Health Team offers many group-based health programs. For example, you can learn to manage stress or learn to cook healthy meals with other patients. We also work with many other organizations in the City of Hamilton to provide care to you.
