About this service
The Primary Care team consists of physicians, nurse practitioners, registered nurses, and a registered practical nurse. We provide holistic primary health care to our clientele taking into consideration the patients physical, emotional, mental and spiritual aspects. The client receives regular ongoing health care which includes health promotion, disease prevention, and health maintenance. The client is considered a partner in his or her own health and plays an important role in the care plan, which enables him to be the best that he or she can be. We monitor chronic illness, as well as see people on an urgent basis for episodic illness. Referrals are made to specialists by the physicians and the nurse practitioners if required.
