About this service
Heart Failure Disease Management Program Heart failure is defined as the inability of the heart to pump adequately to meet the needs of the body. The team in the Heart Failure Disease Management Program provides comprehensive care for heart failure patients throughout the course of their treatment. The program is one of very few in Northeastern Ontario. We also have a new Cardiac Amyloidosis Clinic that screens and treats patients for this rare disease that leads to heart failure. We offer outpatient consultation and follow-up for patients either in-person at Health Sciences North or via videoconference (Ontario Telemedicine Network) if the patients condition warrants.
