Geriatrics We provide comprehensive geriatric assessments for high risk geriatric issues such as falls/mobility; memory loss; depression; medications (polypharmacy). We focus on three key areas with these assessments: function; cognition/mood; and nutrition. We identify common geriatric conditions (orthostatic hypotension etc) and mitigate risks. In order to enhance access to care for vulnerable at risk patients we offer home visits as well as group presentations on managing medication effectively and safely. We promote the use of community supports to help maximize function and independence within the community. We work specifically with onsite CCAC Care coordinators to provide joint assessment and reduce assessment burden on patients. OUR FALLS PREVENTION PROGRAM IS CALLED STAND UP! The goal of the program is to prevent falls and fractures among seniors who are concerned about falling. The program lasts 12 weeks and consists of three components: group exercises, exercises at home and education on falls prevention. The goals of the program are to: improve balance and leg strength; develop skills to make adjustments in the home and adopt safe behaviours; enhance a feeling of effectiveness related to falls prevention; help maintain bone density in sites that are vulnerable to fractures; and encourage regular physical activity.
