Geriatric Outpatient Rehabilitation Service

All patients will receive a comprehensive geriatric assessment in order to determine rehabilitative goals. Based on individual needs, patients will participate in rehabilitation with the interdisciplinary team 2-3 times per week for an approximate duration of 4-8 weeks. Geriatric rehabilitative interventions are delivered by a specialized interdisciplinary team to address geriatric syndromes and functional decline using a holistic approach. Moderate to high intensity intervention is provided and progression is based on the patient goals. Services are provided individually or in groups and are scheduled according to the patients therapeutic needs in order to minimize appointment burden.

Older adults, 65 years of age or over, whose health and independence are at risk due to recent functional decline or impairment, falls or increased risk of falls, activity limitations, or de-conditioning as a result of an acute or sub-acute health condition, pain, injury, hospitalization or surgical procedure. Patient has been identified as having restorative potential: The patients condition is likely to undergo functional improvement and benefit from interdisciplinary rehabilitative care or s/he requires rehabilitative care to prevent functional decline. The patient is able, and willing to participate in and benefit from rehabilitative care; and The patient has identified goals that are specific, measurable, realistic and timely. Patients must be medically stable Patients must be living in the community (not long term care)
