About this service
Connects you with health services and information from the time of diagnosis and through entire progression of the disease. All First Link Care Navigators are registered professionals (Registered Nurse, Registered Practical Nurse, Registered Social Services Worker or Registered Social Worker). Cognitive assessments are also available through our First Link Care Navigators. Results are shared with your family health care provider for interpretation. Referrals are not needed and may be done in home or at the Alzheimer Society. Click here to access our Referral Form Behavioural Support The Behaviour Support Facilitator works with families of the person living with dementia who demonstrates responsive behaviours, providing them with supportive strategies.
