Community Reintegration and Housing Support As a member of the Homelessness Network-Reseau sans abris-En dass qwa uk Mann we nod ma geh win of the City of Greater Sudbury, the Community Reintegration and Housing Support program is dedicated to working collaboratively with individuals and communities to ensure that every door leads to services and to alleviate barriers for homeless and at risk people. Housing and reintegration support services are available for women in the community and those released from correctional facilities to find, secure and maintain suitable housing. Needs Assessment Meet with women in the community as well as women in correctional facilities to develop housing strategies and reintegration plans to facilitate transition from jail to the community, preventing a cycle of homelessness and/or re-incarceration. Assistance in Obtaining Housing Support women through the implementation of their housing strategy and/or reintegration plans once released from correctional facilities and while they are temporarily housed in emergency shelters or alternate accommodations, by providing housing listings and assistance to find and secure housing, and assisting in acquiring the necessary household items to live independently in their own homes. Case Management & Outreach Case management is a vital aspect of the housing program, in which workers provide support, advocacy and accompaniment to community appointments, initiate referrals to other programs and services, work collaboratively with other outreach service providers to maximize the services for homeless people and provide ongoing outreach support and advocacy to women after permanent housing is established in the community.
