The Life Long Care program is available to all aged community members who are interested in improving their quality of health and promote independent living. Our aim is to prevent isolation and encourage socialization. We advocate in navigating the health system. Our programs and services are delivered through a culturally appropriate framework and we understand that each community member has their own unique needs.
