About this service
The Assertive Community Treatment Team is a community based, outpatient treatment program of the Cornwall Community Hospital. It provides services to residents of Stormont, Dundas and Glengarry counties and Akwesasne who have a serious mental illness and require ongoing intensive treatment. The multi-disciplinary team operates as a "stand alone" service offering support and assistance in all aspects of daily living such as housing vocational, financial, legal and medical issues. Team members work with clients to develop and implement client-centered recovery and rehabilitation care plans ensuring clients access the appropriate level of services needed.
