About this service
Sometimes people who have been hospitalized are unable to be discharged. This can happen if they were homeless prior to entering the hospital, or if they lost their housing while hospitalized.
When this happens, we offer specialized transitional housing that includes individualized supports, so people can safely return to living in the community
For clients with less demanding challenges, we offer medium-support and low-support housing through LOFT-owned-and-operated apartment buildings and shared homes. These options are ideal for clients who prefer an independent, or nearly independent, living environment. We also offer ongoing support to clients living in apartments of their own
Shared supportive housing is available for young people experiencing their first psychotic episode, usually between the ages of 16 and 26. These young people receive specialized, recovery-focused care while remaining close to home and the natural support system provided by their family, friends, work and school
