About this service
Our Positive Service Coordination program provides intensive one-on-one case management services to people who are homeless and living with HIV/AIDS, mental illness and addiction, and have experienced a physical or mental health crisis. Currently, the program helps more than 70 people a year leave the street, stabilize, take control of their mental and physical health, obtain affordable housing and recover their dignity and independence.
