About this organization
The Bridge Integrated Care Hub is a low-barrier, HART Hub-funded initiative led by a collaborative partnership of local health and social service organizations committed to supporting individuals experiencing homelessness and other vulnerable populations. Through a one-door, one-roof approach, The Bridge provides access to basic needs and essential services including food assistance, primary health care, mental health and addiction counseling, system navigation, advocacy, and connections to housing and social supports. Provincial HART Hub funding from the Ministry of Health, along with operating funding from Hastings County, also enables community outreach and rapid response teams to rural and underserved areas and the creation of supportive transitional housing units. Eligibility Criteria: Available to guests accessing services at The Bridge Integrated Care Hub. Hours: Monday: 8:30am - 7:30pm & 8:30am - 7:30am Tuesday: 8:30am - 7:30pm & 8:30am - 7:30am Wednesday: 8:30am - 7:30pm & 8:30am - 7:30am Thursday: 8:30am - 7:30pm & 8:30am - 7:30am Friday: 8:30am - 7:30pm & 8:30am - 7:30am Saturday: 8:30am - 7:30pm & 8:30am - 7:30am Sunday: 8:30am - 7:30pm & 8:30am - 7:30am (613)-210-1416
