About this service
This 40 bed transitional unit cares for patients who have a firm discharge destination but face a temporary obstacle, such as awaiting home and community care services or slow stream rehabilitation and convalescent care. Focused, short term supports will enable these patients to be more quickly and safely discharged. Patients are transferred to this unit one a discharge plan is in place. This unit does not accept direct admissions from RVH Emergency department. Our healthcare team consists of physicians, registered nurses, registered practical nurses, patient care assistants, dietitians, pharmacists, physiotherapists, occupational therapists, a discharge planner, social worker, a respiratory therapist, a speech language pathologist, a chaplain, unit clerks, and volunteers. Each member has a specific role to play to ensure optimal health and seamless transitions
