About this service

Sometimes people who have been hospitalized are unable to be discharged. This can happen if they were homeless prior to entering the hospital, or if they lost their housing while hospitalized. When this happens, we offer specialized transitional housing that includes individualized supports, so people can safely return to living in the community.
