About this service
This program provides post-release services to individuals upon discharge from institutions and accelerates the reintegration of released individuals into the community. Together with the client, our Community Reintegration Worker (CRW) completes an assessment to identify the client's needs. The CRW then facilitates access to specialized community-based support programs and prevention services to help individuals successfully reintegrate into the community.
