About this service
Transition Support Services are available to youth and young adults who are attending or who have recently left high school. The Transition Coordinator acts as a support and provides resources to the person, their family, schools and other community agencies. Skills and abilities are assessed, interests are explored through the Individual Support Plan to enhance the persons goals of decision making, problem solving and daily living skills.
