About this service
The Family Home Program provides an accommodation option for children and adults with an intellectual disability who for a variety of reasons cannot or choose not to live with their family. The Family Home Program is designed to enable a person to live as an integral member of an alternative family setting and is supported by an approved Family Home Provider. Wherever possible, the persons relationship with his/her natural family is encouraged and supported by the program. The program takes a person-centered approach to providing supports and services that allow for the attainment of Personal Outcome Measures. All placements are monitored and supervised by a social worker to ensure the needs of the person are being met. The Family Home Provider receives on-going training and support from the program social worker.
