About this service
The Barrie & Community Family Health Teams Diabetes Program is a multidisciplinary team approach to helping patients with diabetes better manage their disease. Our goal is to improve health outcomes for adults living with Type 1 and Type 2 diabetes by providing education that supports individual learning and patient self-management. Our team consists of Certified Diabetes Educators (RNs, RDs and a Pharmacist-with advanced knowledge and training in diabetes management), who work closely with Physicians, Nurse Practitioners and Specialists. This comprehensive program is available to all patients with a Family Health Team Physician. Family Health Team patients are referred to the Diabetes Program by their Family Physician or Nurse Practitioner.
